Overview

VANCOMYCIN

It also prevents the formation of cell walls.

Mechanism of Action

  • It affects the synthesis of cell membranes in organisms that reproduce by interfering with it.
  • Numerous gram-positive bacteria, such as group A beta-hemolytic streptococci, staphylococci, pneumococci, enterococci, clostridia, and corynebacteria, are susceptible to this substance. Mycobacteria, fungi, and gram-negative organisms are exceptionally resistant.

Indication

  • Infected with MRSA
  • False Membrane Colitis
  • Endocarditis

Dose

(offered in 500 mg and 750 mg vials)

  • Adult
    • Daily doses of 500 mg–2 g in 3–4 divided doses, and
  • Children
    • 40 mg/kg (maximum 2 grams) per day

Contraindication

  • Hypersensitivity

Side effects

  • Nephrotoxicity
  • Ototoxicity
  • Hypokalemia
  • Blue Neck

Nursing Consideration

  • Throughout the duration of the drug administration, continuously monitor blood pressure and heart rate.
  • Regularly check hematologic tests, kidney and liver function tests, and urinalysis results.
  • In individuals older than 60 years, babies, and newborns should have their kidney function monitored.
  • Test your hearing. The auditory branch of the eighth cranial nerve may be damaged by drugs.
  • Give via intermittent IV infusion for at least 60 minutes, or for no more than 10 mg per minute, whichever is longer.
  • It should not be taken without a prescription from a doctor.
  • It might make patients allergic. so if it does, let your doctor or pharmacist know.
  • Without laboratory proof, the medication shouldn't be taken.
  • Give medications continuously
  • Keep an eye on your urine output
  • Nursing mothers shouldn't take vancomycin because it could pass through their milk.

CLINDAMYCIN 

Mechanism of Action

  • It prevents 50s ribosomes in bacteria from synthesizing proteins.

Indication

  • Deadly Susceptible Infections, such as
    1. Respiratory
    2. Skin and Soft Tissue
    3. Septicemia
    4. Intraabdominal
    5. Female Pelvic or Genital
    6. Bone and Joint.

Dose

  • Adult
    • Serious
      • 150-300mg qid
    • More Severe
      • 300-450mg qid.
  • Child
    • Every six hours, take 3 to 6mg/k*g.

Contraindication

  • Hypersensitivity

Side Effect

  • Nausea
  • Diarrhoea
  • Vomiting
  • Uncomfortable stomach
  • Oesophagitis
  • Rash
  • Jaundice
  • Neutropenia
  • Thrombophlebitis after IV injection.

Nursing Consideration

  • Consider the allergic past-life information.
  • Obtain a sample for culture and look at the sensitivity profile.
  • Nurses are encouraged to administer this medication before to a meal.
  • It is important to periodically have a liver function test done because drugs can either cause or impair liver function.
  • Diarrhea and hepatic/renal problems could result from it. If so, the only effective ORS is
